Financial Policies
Insurance
It is important for you to be an informed consumer, who understands the specifications of you insurance policy. Your health insurance policy is a contract between you and your health insurance company/employer.

You will be required to provide your current and accurate insurance card(s) at the time of each of you visits. We will scan a copy of your insurance card(s) at the time of your visits. Any co-payment amount will also be due at the time of the visit.
If you do not have your current insurance card, you will be asked to sign a waiver. If you fail to provide the card before the statements are generated, you will be billed for the full amount of your visit.
Mitra Arthritis and Osteoporosis center is a participating Medicare provider. Patients are responsible for their Medicare co-insurance, deductibles, and any services rendered that are not covered by medicare.
In order to see a specialist, some insurance plans require a referral from the Primary Care Physician or a pre-certification before treatment can be rendered. It is the patient's responsibility to ensure we have this referral or pre-certification prior to the visit. If we do not receive the necessary referral or pre-certification, the patient will be responsible for payment or will need to reschedule their appointment.
Self Pay
Mitra Arthritis and Osteoporosis Center does not accept patients who are self pay.
Non-Covered Services
Some services we provide may be deemed not medically necessary by your insurance carrier
or not a covered benefit. The patient is responsible for payment for all services not covered by insurance.
Payment Plans
In the event of extreme financial hardship, we will be more than happy to come to a mutually
agreeable payment plan. Payment plans may only be made by contacting the billing service
at 724-602-4424. Please be advised that if your account has already been turned over to collections, no payment plan may be made.
Failure to Pay your Balance
Patients who ignore statements and fail to pay their balances risk negative credit rating and
possible dismissal from the practice. Past Due accounts may hinder your ability to schedule
appointments or receive approval for refill requests.
Laboratory Services
Some services, such as labs, require that the specimens be collected at a laboratory for
collection and processing.
Laboratory charges are billed separately and are not included in your bill from Mitra
Arthritis and Osteoporosis Center.
Radiology Services
We do not do x-rays at Mitra Arthritis and Osteoporosis Center. Charges for x-rays done are billed
separately and are not included in your bill from MAOC.
Missed Appointment Fees
Mitra Arthritis and Osteoporosis Center may charge a fee for missed appointments when the patient fails to give appropriate notification. A cancellation notice must be received twenty - four (24) hours in advance of the scheduled appointment. A forty dollar ($40) charge may be applied for failure to meet this requirement.
